The second-generation Hilux Surf was released in May 1989. As the changes in the North American import tariffs eliminated the advantage of 2-door models, the second-generation Surf added a more practical 4-door version. Although the Hilux pickup truck remained to be used as the base model, the second-generation model came with a regular steel wagon body instead of just mounting a fiberglass-reinforced plastic (FRP) canopy on the bed of a pickup. As a result, its cabin became more like that of a passenger car.

The principal body type was a wagon, and the van model was available only with a 4-door body and a diesel engine. The engine lineup included a 2-liter gasoline unit, a 2.4-liter turbo diesel unit, and a 2.8-liter diesel unit (dedicated for the van). The transmission was either a 5-speed manual or a 4-speed automatic. The rear suspension continued to use a rigid axle design, although the leaf springs were replaced with a 4-link coil spring system to improve stability and ride comfort.

Standard-equipped on all models was the automatic disconnecting differential (ADD), which could automatically switch between 2-wheel-drive and 4-wheel-drive. A 3-liter V6 gasoline engine was made available in August 1990, followed by a 3-liter turbo diesel engine in August 1993.
